Most heavy nights need water, sleep and patience. A small number are a medical emergency, and the difference is not how much somebody drank — it is how they are breathing and whether they can be roused. Alcohol poisoning treatment in Pattaya is available at any hour, and nobody here is interested in lecturing anyone.
The signs that mean act now
Call 1669 or come in immediately for any of these:
- Cannot be woken, or only briefly and then drifts off again
- Breathing slowly, irregularly, or with long gaps
- Skin cold, clammy, pale or bluish, especially lips and fingertips
- Repeated vomiting, or vomiting while barely conscious
- A seizure
- Confusion so severe they do not know where they are
- A head injury on top of the drinking
Alcohol depresses the reflexes that protect the airway. That is why “let them sleep it off” is the advice that kills people — the danger is inhaling vomit, not the alcohol itself.
While you wait, and the recovery position
Put them on their side, not their back. Bend the upper knee forward to stop them rolling over, and tilt the head slightly back so the airway stays open.
Stay with them. Somebody who seems to be sleeping normally can deteriorate within twenty minutes, and blood alcohol keeps rising for a while after the last drink as the stomach continues absorbing.
Keep them warm. Do not give coffee, do not walk them around, and do not make them vomit — none of those help and the last is dangerous.

Assessment of airway, breathing, circulation and conscious level. Blood sugar, because alcohol causes hypoglycaemia and low blood sugar looks identical to being very drunk — that is a genuinely important distinction and a quick one to make.
Intravenous fluids, anti-sickness medication, and thiamine where it is warranted. Monitoring of oxygen levels and observation until it is clear things are improving rather than worsening.
And a look for the things that hide behind drunkenness — a head injury, a bleed, an infection, or drugs taken alongside. That is the part that matters most and it is the reason not to manage this in a hotel room.
What is easily mistaken for drunk
Low blood sugar, particularly in a diabetic. A head injury from a fall nobody witnessed. A stroke. An overdose. Meningitis. Severe dehydration with heat illness.
Every one of those has been dismissed as alcohol by people who meant well, and every one is time-critical.
The rule of thumb worth remembering: somebody who is getting worse hours after their last drink is not simply drunk. Alcohol levels fall over time. Deterioration means something else.
Drink spiking, and being honest about it
It happens, and people are often reluctant to raise it. Suspect it if the level of impairment is far beyond what was drunk, if it came on very suddenly, or if there is a gap in memory that does not fit the amount.

Tell us plainly. Not to get anybody into trouble — we are not interested in that and nothing is reported — but because it genuinely changes treatment.
Alcohol with sedatives or opioids multiplies the effect on breathing. Stimulants mask how drunk somebody is and add their own problems with heart rhythm and temperature. Alcohol with paracetamol matters for the liver, and paracetamol overdose is the quiet one, because it feels survivable for a day and then is not.
A person who cannot tell us is a person we are treating with one hand tied. If you brought them in and you know, say so.
The Pattaya specifics
Buckets and shared drinks make it very hard to know how much anybody has had. Local spirits are frequently stronger than the equivalent at home, and measures are not measured.
Heat compounds everything. Drinking through a hot day dehydrates far faster than the same night at home, and heat exhaustion and heavy drinking arrive together often enough that we look for both.

If you are the friend
You are doing the right thing by not leaving them. That single decision is the most protective factor in the whole situation.
Bring their phone, passport and anything they might have taken. Note roughly what and how much, when they last seemed normal, and whether they hit their head. Those three facts are worth more than any test we can run.
And do not worry about overreacting. Being told somebody is fine is a perfectly good outcome, and we would far rather see ten people who did not need it than miss one who did — see emergency medical care.
The morning after, and when it is not just a hangover

What is not ordinary: vomiting that continues past midday, vomiting blood or anything like coffee grounds, severe abdominal pain, confusion, chest pain, or being unable to keep any fluid down at all.
Repeated retching can tear the lower oesophagus, and severe upper abdominal pain radiating to the back can be pancreatitis. Both are alcohol-related, both need seeing, and neither is a hangover to be waited out.
